Responsibilities

While no two days are exactly the same, here are some things you will be responsible for:

  • Performing personal care tasks, including feeding, ambulation, and medical monitoring
  • Assist with fostering positive relationships between individuals served and their housemates
  • Ensuring client safety and maintaining a safe environment
  • Encouraging self-help activities
  • Accompanying clients to scheduled appointments

Qualifications

  • Must be 18 years of age or older
  • Must have a valid driver’s license
  • Ability to work in a group home, home-like setting
  • Ability to communicate (verbally and written) with all levels of personnel, internal and external

We help our clients remain safe and comfortable with dignity, in their own homes or provide safe, home environments. We are now focused on furthering proactive and client-centered care by bringing primary care into group homes where clients live. For example, we achieved a significant reduction in hospitalizations for our clients in Ohio who received home-based primary care (HBPC) in addition to their regular support services in the home. Clients who received HBPC experienced a 57% reduction in hospital admissions during the observed period compared to a control group receiving traditional primary care services.
